Update Contact Information in Jupiter
(REQUIRED: Due 8/11)
Please log into Jupiter and click on “Settings” and confirm that the listed contact information is correct. If you do not remember your password or are having difficulty logging in, click on “Help Me Login” and enter the primary email address used when you applied to RMHS. If you are unsure what email address was used, please contact the RMHS staff at info@RiverMontessoriHS.org.
Fill Out Alternative Pick Up Location Form in Jupiter (REQUIRED: Due 8/11)
At 5:00 p.m. the RMHS campus closes to student use and all RMHS students must exit the building and outdoor school property unless participating in an extracurricular activity. Families are required to indicate a default alternate pick up location that RMHS staff can direct the student to walk to. Suggested examples include:

Fill Out Health Information Form in Jupiter (REQUIRED: Due 8/11)
Please log into Jupiter and click on “Forms” to access the 2021 - 2022 Student Health Information Form. This form provides important medical contact information as well as permissions for what medications may be administered by school staff.
Proof of Covid-19 Vaccination
(OPTIONAL)
In order to be eligible to attend RMHS without wearing a mask students must be fully vaccinated against Covid-19. To submit an image or scanned copy of your student’s vaccine documentation, log into Jupiter and select “Forms” and “Proof of Covid-19 Vaccination.” For questions, please contact RMHS staff at info@RiverMontessoriHS.org or call/text 574.400.5663.
Download and Read “City as Classroom” Guidelines (REQUIRED: Due 8/11)
Please read the linked “City as Classroom” guidelines. These are referenced as a part of the “Required Consents” in the RMHS General Waiver which must be signed in order for your student to attend RMHS. Hard copies of the “City as Classroom” Guidelines will be available at Parent/Guardian Orientation.
Download and Sign RMHS General Waiver (REQUIRED: Due 8/11)
In order for your student to attend RMHS you must sign the RMHS General Waiver. Hard copies of the waiver will be available at Parent/Guardian Orientation.
Download and Sign “Laptop Use Agreement” (REQUIRED: Due 8/11)
Students at RMHS are assigned their own laptop for use at school. Please download, sign, and return the linked usage agreement. Parents/guardians may provide additional consent for the laptop to be brought home and used outside of school hours.
Arrange Submission of General Vaccination Records (REQUIRED*: Due 8/11)
*Returning Families: Disregard this step if you submitted vaccination records earlier this year and there have been no new vaccinations since that time.
RMHS is required to collect general vaccination records from all students in attendance as well as provide families with information regarding Meningitis vaccines. To view this and other information related to vaccines, click the button below.
Instructions For Obtaining and Submitting Vaccine Records:
(1) Please have your family doctor fax your student's vaccination record to 574-244-8450. This request can typically be made over the phone and does not require a doctor's visit.
(2) If you do not have a family doctor or if your family doctor does not have your full vaccination records, please see the “Obtaining Vaccination Records” .pdf in the folder linked below for steps you can take to secure vaccination records.
(3) If your student is not vaccinated for medical reasons, fax (574-244-8450) or email (info@RiverMontessoriHS.org) a signed letter from your physician indicating which vaccines your student has been unable to receive.
(4) If your student is not vaccinated for religious reasons, please fill out, sign, and return the religious exemption form located in the folder linked below using the fax number or email address listed above.

All required RMHS general and academic supplies are provided for 9th grade and new students.
Download and Sign Prairie Winds Overnight Permission and Waiver (REQUIRED: Due 8/13)
On Thursday, August 19th all students will be participating in an overnight orientation at Prairie Winds Nature Farm. This requires an overnight permission slip as well as a liability waiver specific to Prairie Winds Nature Farm. Hard copies of these forms will be available at Parent/Guardian Orientation or they may be downloaded and signed in advance.
Voucher Parent Agreement (REQUIRED*: Due 8/13)
*If you receive financial aid from RMHS you are required to submit this agreement unless prior arrangement has been made.
A signed copy of the agreement linked below must be submitted to RMHS in order for the school to apply for a voucher on your behalf. In addition to this document, there will be additional forms requiring your signature throughout the year. Timely cooperation is essential. Hard copies of this form will be available for filling out and signing at the Parent/Guardian Orientation.
Free/Reduced Lunch Application (OPTIONAL: Due 8/13)
As a part of its accreditation RMHS is required to furnish all families with a Free/Reduced lunch application. However, families are not required to return this form in order to participate in the lunch program at RMHS as it is free for all students. Additionally, all textbooks are included with tuition.
Hard copies of this form will be available for filling out and signing at the Parent/Guardian Orientation.
Join RMHS Remind Text Message Group (OPTIONAL)
In addition to Email, RMHS staff will send out school closure or emergency information via Remind text messaging. All information shared through Remind will also be emailed, so usage of this service is optional.
